Custom Instruction Form

Please fill this out as completely as possible. We will make every effort to initially get back to you within 24hrs to let you know our potential availability. Please note that we can not fill all custom requests due to instructor availability. The more lead time and possible dates you can give us, the better your chances of being able to book a custom clinic. On busier days, we may ask you to provide a vehicle capable of carrying the instructor, students, boats and gear.
